Case Management


The reintegration via the Case Management is possibly the most important aspect of the project, and potentially the most challenging. 

The reintegration team works to reintegrate the children into their family, the school system and the workplace. The team initially works with the child to define the family history. At the same time visits to the family take place to help the family in solving its problems. Finally, the child is reintegrated in the most appropriate setting (direct family, extended family, guardianship, independent life). The team follows up on the child's situation regularly in close collaboration with the NCCM.

FACE tries to find solutions and ways to ensure sustainable family life, rather than support the pattern of the child constantly leaving and returning home.
